wang-bin b971c51180 allow null a/v thread. demux thread exits only when a/v thread exits 
TODO:
if demuxer read the eof packet, then it will not get that again if seek
again, why? miss resetting something?

wang-bin ee3181af32 fix crash and video blocking when seeking 
avcodec_flush_buffers() immediatly after seeking may cause a crash or
blocking the thread, but
i don't know why.
TODO: flush in AVThread when got a flush packet

FIXME: the clock will keep ExternalClock type even if play a new stream
with audio stream

1. can not seek backward.
2. frames in the end can not be played correctly

wang-bin 0b741e3300 QtAV_Compat.h: fix compile failure with gcc. Only included by .cpp 
the macro av_err2str() in ffmpeg's error.h results in a "taking address
of temporary array" error using gcc in linux. MinGW is ok. Don't know
why.
QtAV_Compat.h must include libav/ffmpeg headers, so it can't be included
in a public header.
