Fix modification of monitored items
...
If a client handle is modified after creation, the new value is used in
data change notifications from the server. If the new handle doesn't match
the initial handle, the client is unable to dispatch the data change
notification to the callback associated with the monitored item.
Client handles supplied by the API user during monitored item creation
are replaced internally by a counter value and there is no way to retrieve
the client handle for a monitored item after its creation.
Setting the client handle from the monitored item id before sending the
modify request to the server solves that problem.

Added callback function to get client config from config plugin
...
The server implementation needs to initalize a client when it
shall register itself to a discovery server. To avoid including a
private header file of the plugin a callback function has been
declared in the server header file to be implemented by the plugin
implementation. This callback function are called by the server
when it shall initialize a client.

SecurityPolicy Interface change ( #1613 )

* Always use functions to get sizes and lengths.
This makes for better encapsulation and unifies the interface usage for
asymmetric and symmetric modules
* Add nonce length to SecurityPolicy and use it.
Previousely the key length was used as nonce length, which was
incorrect.
This only worked for basic128rsa15.
* Add certificate signing algorithm to interface
* Remove unnecessary functions from SecurityPolicy interface
* allow session to switch over to a new securechannel

added "UA_Server_addMethodNodeEx" to public API:
...
- allows direct specification of requested node IDs for method nodes so that there is no requirement of public "UA_Server_addMethodNode_finish" any more
- old API and kept untouched (backward compatability)
- redirected "UA_Server_addMethodNode(_finish)" to use new "UA_Server_addMethodNodeEx(_finish)" (with UA_NODEID_NULL as requested node IDs which keeps old behaviour)
