Without encryption support, the #None endpoint offered x509 and password
tokens that ActivateSession always rejects. The endpoint check now runs in
every build. It removes x509 tokens without the password warning.
Use pointer-sized atomic run flags in threaded fixtures, including dynamically
allocated discovery and browse flags. Pointer sizing supports the MSVC atomic
implementation as well as C11 and GCC atomics.
Serialize the client error-injection test with the server network callbacks
before accessing the internal session and shared send buffer.
This tests that a server certificate is sent if it is necessary for
the endpoint. In this particular example, the UserNameIdentityToken
uses a channel with None security policy but the token specifies a
security policy separately to encrypt the password.
