feat(server): Extended the access control plugin with check for browsing nodes
...
The PermissionType (Part 3) defines 17 permissions. Currently the
access control plugin can control all but 2 of them. Those are browse
and ReceiveEvents. This change adds check for browse permission of
nodes.

This commit drops the 'ua_' prefix and changes the public includes
to the form `#include <open62541/server.h>` instead of the old
`#include <ua_server.h>`

AccessControlContext not initialized
...
When calling UA_AccessControl_default with no username password (usernamePasswordLoginSize = 0) context->usernamePasswordLoginSize is not initialized. This causes a crash when deleteMembers_default is called.
