Remove the Policies section, Human Assist section, related credential
hint in Core Workflow, the References table entry, and the
references/policies.md file.
Rewrite Policies section with explicit field definitions (enabled/trigger/action/note),
available actions table, continuous evaluation rules, and policy discovery mechanism that
suggests saving new policies after non-policy-triggered human assists.
Add human-assist-url vs conversational confirmation guidance to prevent bypassing
the policy discovery flow when user confirmation is needed on page content.
Validated with 31 test scenarios across 10 categories (all passing).

Introduce trigger-action policy system for browser automation:
- Three preset rules: credential-login, captcha-unsolvable, operation-stuck
- User-controlled policy file (references/policies.md) with enable/disable support
- Human assist workflow via human-assist-url command
- Explicit safeguard: Agent cannot auto-create/modify/delete policies
Validated with 170 test scenarios across 11 real-world categories
(130 policy-decision + 40 write-behavior), achieving 100% lenient accuracy.
